Fees

All registrations must be accompanied by the appropriate fee:

Initial registration for resident physician, assistant resident physician, intern, or fellow in fellowship training in a teaching hospital in this state as defined in s.408.07(44) or s.395.805(2)

Initial Registration for resident, fellow, or intern: $200.

Initial registration for House physician: $300.

House physician renewal registration fee: $200.

Make certified checks or money orders payable to: “Department of Health” (DOH); or pay by credit card when applying online.
